I hereby give permission for Sifil AFC to use my child’s image, name, and video footage for the following purposes:

• Social media platforms (Facebook, Twitter, Instagram, etc.)
• Club website and newsletters
• Promotional materials for the club

I understand that:

• The content may be shared publicly on social media.
• My child’s name may be mentioned alongside any images or videos.
• I can withdraw my consent at any time by notifying the club in writing.
• Details of this form, along with any documentation for the child, could be stored on the clubs nominated document management system.
